Grade 7 Physicals

                                                                                                              February 8, 2017
Dear Parent/Guardian:

Your seventh grade child is scheduled to have a school health examination under the authority of Massachusetts General Laws, Chapter 71, Section 56.  All children in grades three, seven and ten are required to have a current, completed physical exam on file in their school health record.

Your family physician has comprehensive knowledge of the health status of your child.  Therefore, we strongly feel that your own physician should complete this examination.  If this exam is completed by your physician, please return a copy of the completed physical form to the school nurse.

If your child is unable to see a physician of their own, we are offering incoming seventh grade physicals at the Memorial Middle School in the spring by the school physician, Dr. Martin Iser.  There will not be a make-up session.  This physical may include any of the body area, such as head, chest, abdomen, external genitalia, and extremities, as well as other areas listed on the attached form.

Parent/Guardian must fill out the medical questionnaire on the attached form, sign the permission slip below and return both to the school nurse by Friday, March 3, 2017.

                                                                        Sincerely,

                                                                        Sharon M. Striglio, R.N.
                                                                        School Nurse

I give permission for my son/daughter, ________________________, to have their physical examination by Dr. Martin Iser at the Memorial Middle School.

My son/daughter, ______________________, will be examined by our family doctor on _____________________.  I will return the completed form to you at that time.

MySchool Bucks!

No student should have a negative meals balance - Keep your student’s meals account balance current by setting up on-line alerts when their account reaches $10.00 or below.  You do not need to pay on-line to receive the alerts
It’s very easy – go to www.myschoolbucks.com to create your account.  You can find your students ID in ASPEN on the Family Page, click “Details” – the number listed under “Local ID” is the student number.  Follow the prompts and be sure to add each student individually.  Choose to receive notifications by text message and/or email.
If you do choose to pay on-line, there are a couple of options – set up auto pay for a specific amount or pay on-line when you receive the alert.  The student will always have money available on their account.
If you would rather not pay on-line, that’s fine too.  When you receive the notification, send in cash or a check (payable to Hull Public Schools) with your student and they can pay the cashier in the cafeteria. It’s that easy!
Student’s who reach a negative balance of $5.00 or more will receive a notice from the school and an alternate lunch will be given until the balance is paid in full.
If you think that your family may be eligible for free or reduced priced meals, please fill out the meals application and send it into school with your student.  A copy of the application can be printed from the Hull Public Schools website or you can pick up a copy in any school office.  You will receive a letter from the school regarding the decision within a week of submission.  If you don’t receive this letter, do not assume you are approved; contact your student’s school.

If you have additional questions you may contact Patty Gardiner at HPS Administration, 781-925-4400  X1112.

Congratulations to Maeve!

This past Saturday, February 4th, the Southeastern Massachusetts Schools Bandmasters Association held auditions for their Festival Concert Band.  Memorial’s very own 8th grader, Maeve Donohue, auditioned and was not only accepted into the ensemble, but among the 42 trumpet players auditioning in Southeastern Massachusetts Maeve was ranked number one!
In addition to this recent accomplishment, Maeve also auditioned for a seat with the Festival Concert Band organized by the Southeastern District of the Massachusetts Music Educators Association in early December.  She, too, was accepted with outstanding marks.  The audition process for both of these music associations is quite challenging and Maeve’s success is a testament to her hard work and dedication.
Maeve’s performance with the SEMMEA Concert Band (Jr.) will take place on Saturday, March 4th at Nauset Middle School in Orleans, MA.
Maeve’s performance with the SEMSBA Concert Band (Jr.) will take place on Saturday, May 6th at East Middle School in Braintree, MA.
Congratulations on your outstanding achievements and best of luck with your performances!!!
