Attention Parents of Students in Grade 8:
All incoming 9th graders who are interested in playing sports at the high school need a copy of a recent physical (within the last 13 months) on file in the Nurse’s Office. If you would like your child to have a physical this spring by the school physician, print a copy of the permission slip and physical form and return it to the school nurse by May 9th.
Attention Parents of Students in Grade 7:
All 7th grade students are required to have an updated physical examination on file in the Nurse’s Office. Please send updated forms to the school nurse. The school
physician, Dr. Iser, will be at school this spring to perform
physicals on students here at the Memorial Middle School for those students who are unable to see their own physician. Permission slips have been mailed home and should be returned by May 9th.
Attention Parents of Students in Grade 6:
All incoming 7th graders are mandated to have an updated physical on file in the Nurse’s Office. Letters were sent home to parents/guardians of students that do not have an updated physical on file. If you would like your child to have a physical this spring by the school physician, print a copy of the permission slip and physical form and return it to the school nurse by May 9th.
If your student has had a recent physical, please send a copy to the Nurse’s Office to update their health record.
